AHS conducted a 2nd qtr focus monitoring in the above rule area, reviewed 2 staff records. 1 of 2 staff records did not have LHPS check off, HCPR, medication clinical skills checklist. 2 of 2 staff records did not have state approved infection contol training. Interview with the Administrtator revealed this information was not onsite but she would obtain the information and forward to AHS no later than tomorrow 12/13/2023 by 5pm.
