Failure to provide supervision to disoriented Assisted Living (AL) resident(s) with disorientation per their assessed needs, current symptoms, and care plan resulted in the elopement of an AL resident with dementia/disorientation into a city neighborhood without the facility's knowledge. A Type A2 Violation citation/CAR was issued to the facility in Personal Care & Supervision 5/15/24.
