AHS made an unannounced visit to the facility. Entrance with the Supervisor in Charge(SIC). Based on observations, interviews and record reviews, the facility failed to perform fire rehearsals quarterly on each shift. A CAR will be issued in rule area 10A NCAC13G.0316(e) within 10 working days.
