AHS made an unannounced visit to the facility. Entrance with the Administrator. Observation of hot water temperatures revealed, hot water temperatures exceed the maximum of 116 degrees.

Observation during a facilty tour revealed the medication prep room was propped open with a broom. The med room had two bottles of medications sitting on the counter, a medication control substance book on the medication cart and a resident's personal information which included FL-2, care plan, LHPS, and physician orders.
