June 23, 2026 Adult Home Specialist initiated a routine monitoring in Management of Resident Funds and Refunds. Information to determine compliance was not available during onsite visit. AHS requested information to be provided by close of business June 24, 2026. Second Request. Nothing Received June 25, 2026 AHS made phone call on June 25, 2026 for a third request for information to complete 4th quarter monitoring. Nothing Received June 29, 2026 AHS will issue a CAR with 10 working days ( July 14, 2026) for the facility’s failure to cooperate in determining facility compliance. 10A NCAC 13G .0406 (a)(10)
