Narrative

AHS made an attempt to conduct a 2nd quarter focus routine monitoring at the facility. No one was available to obtain access to staff files. AHS collaborated with the Administrator who was 30 minutes away from the facility. AHS notified the Adult and Community Supervisor and followed up with the Administrator via email about the importance of having someone on site to access staff and resident records. AHS will return at a later date to continue the focus monitoring.
